Some patients may require bone grafting, gingival grafting or sinus grafting in order to augment bone or reshape tissue.
Bone grafting is necessary when bone width or height is inadequate for receiving an implant. Sinus floor elevation is an augmentation technique used when there is insufficient vertical bone for implant placement in maxillary posterior areas.
Bone grafting is carried out under local anesthesia & can be taken from patient’s own body, bovine grafts available in the market, & synthetic grafts which contain hydroxyapatite crystals, tricalcium phosphate. Bio-Oss & Bio-Gide are widely used as dental xenograft materials. Bone grafts are also available in particulate & block forms.
In autogenous graft, small bone block is taken from the patient’s own body & is placed on to the deficient bone site & is stabilize using titanium screws. At times, resorbable collagen membrane is placed after grafting material placement just to hold the graft at the site.
Depending on the situation, bone grafts may be placed at the same time as an implant, or before the implant. While it is more convenient to place the implant & graft at the same time.
Here we follow proper treatment guidelines set out for the treatment. If the implant is placed too soon after the graft is placed, the graft will not have had enough time to heal & become solid. If the implant is placed too long after the graft is placed, resorption & melting of the graft may occur with loss of bone volume. So considering all these we schedule implant placement such that where sufficient healing, but minimal resorption has occurred.
